A centralized process and single point of access to mental health intensive case management services for residents of Hamilton.

The 3 organizations that provide Mental Health Intensive Case Management Services in Hamilton are:

  • Canadian Mental Health Association (CMHA)
  • Community Mental Health Promotion Program (CMHPP) - City of Hamilton
  • Hamilton Program for Schizophrenia (HPS)
Eligibility
To be eligible for this service, the person referred needs to be: Living in Hamilton 16 years of age or older Diagnosed with a serious and persistent mental illness or concurrent disorder Under the care of a physician (some services require a Hamilton physician) Willing and able to participate in working on goals
Application Process
Referral can be made to this program through healthcare professionals, community/social service agencies, family/friends, or self-referral. Referral forms can be found online.
