Case Management
- conducts assessments, develops care and safety plans, provides supportive counselling to individuals and their families
Support within Housing
- provides housing units at a subsidized rate and case management services to clients and their families
Coordinated intake through The Access Point (see separate entry)
- Eligibility
- Individuals 16 years and older with mental health challenges that are seriously impacting their lives
Supportive Housing applicants must qualify for a housing subsidy under the criteria set by the Ministry of Health, and are willing to accept some level of support from the housing provider
Residency Requirements: Must be living/staying or planning to live/stay in Toronto - Application Process
