Description

Case Management

  • one-on-one support for individuals who require individual case management support to live in the community, including links to appropriate services

Supportive Housing

  • congregate living, high support living, addictions supportive housing and independent living
  • case management services
Eligibility
Individuals 16 years and older with mental health and/or addiction challenges that are seriously impacting their lives
Supportive Housing applicants must qualify for a housing subsidy under the criteria set by the Ministry of Health, and are willing to accept some level of support from the housing provider
Residency Requirements: Must be living/staying or planning to live/stay in Toronto
