- This program assists Indigenous, Metis and Inuit youth. It is designed to support the youth while transitioning out of the Child Welfare system into adulthood Youth In Transition Worker assist in long/short term plans and decision making with regards to:
- housing
- education
- health
- mental health
- legal
- employment
- life skills
- personal goals & well being
- Eligibility
- Indigenous, Metis, and Inuit youth, age 16-24 years, who have had a history with the child welfare system, all CAS within Canada
- Application Process
- Referrals can be made by Children's Aid Societies, community agencies, family, peers and/or youth directly
