- Six Nations Band Members - call 519-445-0410 at least 5 days prior to the appointment
New Credit and other First Nations - medical transportation requires prior approval. Requests must be made at least 2 weeks prior to the appointment
Information required:- full name
- date of birth
- complete address and telephone number
- 10-digit status card registration number
- doctor's name and the area of specialty
- date and time of the appointment
- address of appointment
- any other medical transportation needs (i.e. wheelchair, escort)
