  • private driver to/from medical appointments
  • wheelchair transportation
Eligibility
  • Six Nations Band Members living on and off reserve
  • Mississaugas of the Credit First Nation Band Members
  • other First Nations
Application Process
Six Nations Band Members - call 519-445-0410 at least 5 days prior to the appointment
New Credit and other First Nations - medical transportation requires prior approval. Requests must be made at least 2 weeks prior to the appointment
Information required:
  • full name
  • date of birth
  • complete address and telephone number
  • 10-digit status card registration number
  • doctor's name and the area of specialty
  • date and time of the appointment
  • address of appointment
  • any other medical transportation needs (i.e. wheelchair, escort)
