Mental Health Support Services
- Description
Assertive Community Treatment Teams
- client-centered, recovery oriented mental health service offered by a multidisciplinary team
- offering a range of intensive community supports to individuals experiencing mental health concerns and their families, in order to reach their recovery goals.
- program is intended for individuals 16 years of age or older with a primary diagnosis of schizophrenia, bipolar disorder or major depression
- individuals who have high service needs who have not otherwise been successful with office-based services or have had extensive hospital stays.
Case Management Program (16+)
- assistance with skills development, primary health care, employment, accessing housing and income supports, reducing social isolation
Clinical Therapy (18+)
- group-based program for people with serious mental health issues, assists in managing unhelpful thoughts, overwhelming/difficult emotions, and problematic behaviours, and developing and practicing coping skills
Community Connections (16+)
- drop-in hub for individuals, includes free mental health workshops, opportunities for socialization and activities to improve mental health and wellness, and peer led activities in the community
Community Transitions Team (16+)
- aims to create a safe, confidential, and inclusive space to get culturally competent and gender-affirming care to trans and gender diverse people.
HOPE Early Psychosis Intervention Program (EPI) (14-35)
- comprehensive treatment program for people who have experienced a first episode of psychosis
- treatment is provided by a coordinated specialty care (CSC) team, which helps people work toward personal goals and recovery
- helps clients navigate the road to recovery from an episode of psychosis, including supporting efforts to function well at home, on the job, at school, and in social situations
- eligibility will be determined by H.O.P.E. clinical team as per Provincial standards
- admission decisions are based on considerations that include: admission criteria, current caseload status, staff capacity, ability to manage risk in the community
Mental Health & Justice
- case management and court support for individuals with mental health concerns and involvement in criminal justice system
- works with individual and their family to develop support and treatment plan and reduce social isolation
- assist with navigating court system and obtaining appropriate referrals
Support for Depression
- for people experiencing or who have experienced mental illness, includes peer support groups, mutual support and coping skills develop, trained peer volunteer facilitators, and safe, non-judgmental environment
Newcomers Health & Wellbeing (12+)
- provides mental health and primary care services to newcomers in York Region and South Simcoe who have physical and mental health concerns.
Rapid Access Psychiatry
- an evidence-based model of service for adults with mild-to-moderate depression and anxiety, offering different levels of support.
Support for Depression & Support for Anxiety
- support groups for individuals who are experiencing or have experienced depression or anxiety, either on their own or in conjunction with other mental illnesses
- facilitated by trained volunteers with lived experiences of anxiety
Telemedicine
- help for clients, their physicians and healthcare providers connect virtually through the Ontario Telemedicine Network (OTN)
- nursing support is available upon request

- cognitive behavioural therapy (CBT)
- Eligibility
- Programs vary; most programs youth and adults 16 years and older who are experiencing mental health problems; some programs 12 years and older
Mental Health & Justice Program and Youth Court Support: children and adults 12 years of age and older experiencing mental health issues, development disabilities, dual diagnoses, concurrent disorders, acquired brain injury, autism, and/or Alzheimer Disease and/or dementia and criminal justice system involvement - Application Process
