  • Mobile interdisciplinary teams providing mental health and addictions support
  • Addictions treatment bed-based services
  • Mental health and addictions supportive housing
Eligibility
Adults and youth (age 16 years and older) living in Guelph and Wellington County with multiple health conditions, including moderate to severe mental health and/or addictions challenges and experiencing homelessness, being at-risk of homelessness or requiring intensive services to maintain access to housing.
