Services include:
- Mental health and addictions services available on site and through outreach
- Mobile interdisciplinary teams providing mental health and addictions support
- Addictions treatment bed-based services
- Mental health and addictions supportive housing
- Eligibility
- Adults and youth (age 16 years and older) living in Guelph and Wellington County with multiple health conditions, including moderate to severe mental health and/or addictions challenges and experiencing homelessness, being at-risk of homelessness or requiring intensive services to maintain access to housing.
