Clinical Care

  • primary care/family medicine; pediatrics
  • gender-affirming care
  • Geriatric Assessment Intervention Network (GAIN) Clinic

Mental Health

  • brief counselling for individuals, couples and families; group counselling
  • interprofessional care (IPC) mental health team for those with mental health issues and a primary health provider who need additional support temporarily
  • counselling for Indigenous children younger than 12 years and their families
  • Durham Counselling Walk-In Clinic - single session counselling for youth 3-19 years

Wellness

  • nutrition assessment and education; community kitchen
  • wellness sessions such as yoga

Chronic Disease Prevention and Management

  • diabetes; Hepatitis C; harm reduction

Health Education

  • provided through the Learning Hub - in person or virtual
  • for community members, health partners and health professionals

Mobile Clinics

  • primary care mobile clinic - focus on newcomers and/or those without health insurance
  • Hepatitis C and harm reduction mobile service

East Region Virtual Care Clinic (ERVCC)

  • virtual care by nurse practitioner for those without a primary provider or those who cannot access their provider that day
