Eligibility
High-risk seniors, determined by Ontario Health atHome, Central East Area

Supportive Housing Services - residency at either Sunrise Seniors Place or Faith Place, referrals can be made by residents and/or caregivers of residents living in these two facilities

Supportive Housing: contact Sunrise Seniors Place about applying for housing at their facility, call 905-432-8416 or to contact Faith Place about applying for housing at their facility, call 905-576-8619 or visit Faith Place
