Description

Provides support to clients for high-risk transitions to long-term care homes for people who exhibit responsive behaviours

  • transitional lead works with client's family and care providers to develop behavioural care plans as client is admitted into long-term care
  • works with BSO Long-Term Care Home Mobile Team to provide intensive support in first six weeks after admission
