- Description
Program helps to facilitate the discharge of patients ready to leave the hospital who face barriers to a smooth transition from hospital to home.
Services provided by a PSW include:
- picking up medications
- picking up groceries
- light meal preparation
- personal care
- homemaking services
Shortly after the settlement home, the patient will receive a follow-up phone call and will receive any referrals to services needed.
- Eligibility
- Patients who are discharged from hospital to their home. Program is initiated by hospital discharge planner.
- Application Process
- Call or visit Caredove
- Clients will be assessed for barriers to discharge from the hospital and may receive help for a limited period of time with services such as meals, transportation, emergency response, respite, and one-time expenses such as the purchase or rental of mobility equipment.
- Coordinator will visit the home to assess the needs of the client.
