Primary Health Care Team services include:- preventative health care - screening, smoking cessation, teaching and education
- routine physical exams
- cervical cancer screening (HPV testing)
- immunization for children and adults
- assessment and treatment of acute and episodic illnesses
- on-going management of chronic diseases and the development of self-management skills
- pre and post-natal care
- triage service for urgent client medical conditions
- chiropody services
- therapeutic counselling one-on-one or with family
- nutrition assessment and counselling
- health education - healthy lifestyle and diabetes management
- after-hours phone consultation
- information, advocacy, service referral and practical support
- flu immunization clinics for CHC clients
- Access Clinic to provide easier access to newcomers with or without a health card
