Placement Services determine eligibility for admission to long-term care facilities, manage waitlist and admission process for long-term care homes within our region * Support and information is provided to applicants and their families prior to and during the process
- Eligibility
- Must be age 18 years of age or older * Have a valid Ontario Health Insurance (OHIP) Card * Have care needs that cannot safely be met in the community * Care needs that may include 24-hours nursing care and personal care, frequent assistance with activities of daily living, on-site supervision or monitoring to ensure your safety and well-being * Eligibility criteria will be assessed upon initial intake
Residency Requirements: north Toronto (North York), parts of Etobicoke, most of York Region, and South Simcoe County - Application Process
