- Description
Care coordinators assess patient eligibility for home care services, determine care needs * coordinate, monitor and adjust care plan * arrange for palliative care education and direct clinical care in the home including pain and symptom management through medication and other interventions * Care coordinators also make referrals to other community-based palliative care supports such as counseling, day programs, hospice programs, or residential hospice placement
