- Description
A temporary stay in a long-term care home for an individual whose family or caregiver requires a rest or respite from their care giving responsibilities * short-stays are temporary and it is expected that the individual will return home * maximum length of stay is 60 days at a time, up to a total of 90 days in a calendar year, in a long-term care home * long-term care home availability is on a first come first serve basis
- Eligibility
- Available to Ontario residents, 18 years or older who have a valid OHIP card
Residency Requirements: northern Toronto (North York), parts of Etobicoke, most of York Region, and South Simcoe County - Application Process
Call to make a referral or contact your care coordinator if already on service * a care coordinator will visit to assess needs, determine eligibility, manage the application process if eligible, and help explore other options if not eligible
