- Description
Adult day programs (ADP) provide part-day supervised activities in a group setting for dependent adults * services may include social, leisure fitness, or therapeutic activities, personal care and meals * family members and caregivers receive regularly planned respite from caregiving responsibilities.
Individuals who may benefit from adult day program activities include those experiencing:- Acquired brain injury
- Aphasia
- cognitive disorders such as Alzheimer's or other dementias
- development delays
- frailty
- physical disabilities
- social isolation
- Stroke
- Eligibility
- Must be 18 years of age or older * possess a valid Ontario Health Card * have a health need that can be met through an adult day program * Ontario Health atHome care coordinators can help assess the needs of applicants
Residency Requirements: northern Toronto (North York), parts of Etobicoke, most of York Region, and South Simcoe County - Application Process
