- Description
Health services for seniors with complex care needs that may include:
- personal support services (e.g., bathing, dressing, personal hygiene)
- homemaking (e.g., housekeeping, laundry, meal preparation)
- security checks/reassurance services (by telephone or in-person)
- care coordination (e.g., assessment, development of care plans, referral to community agencies)
Services may be provided on a scheduled or unscheduled basis
- Eligibility
- In order to qualify for Assisted Living
Services, you must possess a valid Ontario Health Card * be assessed by a care coordinator * an applicant with care needs that require the availability of 24-hour personal support or attendant services * not require 24-hour nursing services * live in building where Assisted Living Services are provided or within a specific geographic boundary of an approved Assisted Living service provider
