Health care services offered at home, at school, and in the community, as well as access to long-term care and supportive living programs * care coordinators work with people in the community or patients leaving the hospital to determine what help is needed and arrange for care *provides information about other community services and refers when appropriate

Services may include:
  • care coordination
  • nursing
  • physiotherapy
  • occupational therapy
  • nutritional counseling
  • speech therapy
  • social work
  • personal support services
  • school health services
  • mental health and addictions nursing for students
  • palliative care
  • medical supplies and equipment
  • access to long-term care homes
  • access to supportive living (adult day programs, supportive housing, assisted living)
  • referral to other community supports and services

Eligibility
People of all ages who are facing the challenges of age, illness, injury, or disability

Residency Requirements: York Region, South Simcoe County, and parts of North Toronto and Etobicoke
