Central intake for patients with moderate to severe hip, knee arthritis * provides patients with an assessment by an Advanced Practice Provider (APP) within 4 weeks of when the completed referral is received by the Central Intake Program * APP is a Regulated Healthcare Professional such as a Physiotherapist, Registered Nurse, Nurse Practitioner or Chiropractor with additional training in orthopedic care * APP conducts a standardized assessment and based on results, patients will be referred for surgical consultation based on the patient's choice of preferred surgeon, hospital, or next available appointment, or provided with education and non-surgical recommendations, such as changes to physical activity or rehabilitation services, so patients can confidently manage their care * along with care continuum, Primary Care Providers will receive updates on their patient's referral status, assessment and care recommendations
