- Provides medium-term, hospital-level medication management for adults diagnosed with serious and persistent psychiatric illnesses
Services include:- prescription and administration of long-acting injection medications
- assessment and monitoring of symptoms and medication side effects
- education for patients and families
- Eligibility
- Adults 18 years and older living in Halton with serious and persistent psychiatric illnesses (such as psychosis, schizophrenia, mood disorders, anxiety) resulting in functional impairment and significant disability in the daily living
