- Description
- Provides short term support and case management to children, youth and their families as they navigate through the mental health system and transition to longer term services
Services include:- community case management
- collaboration and referral to community partners (connecting children, youth and their families to the most appropriate service provider, liaising with other parts of the health care system, accompanying clients to their appointments with psychiatrists and mental health workers, etc)
- crisis intervention and outreach
- in-school support to children and their teachers
- Eligibility
- Children and youth under the age of 18 years who are at risk for hospitalization or who are transitioning from the inpatient setting or the Emergency Department to outpatient care
