- Description
Provides outpatient care, education and support to children and adults with type 1 and 2 diabetes, prediabetes and gestational diabetes * team consists of diabetes educators (nurse educators and dietitian educators), social worker, kinesiologist (exercise specialist), endocrinologists, nurse practitioner, and support staff
- Eligibility
Adults 18 years and older with pre-diabetes, type 1 diabetes and type 2 diabetes * paediatric (5-18 years old, seeing at Oakville Location only)
Residency Requirements: Milton and area- Application Process
Primary care providers (family doctor or nurse practitioner) or community endocrinologists can refer a patient by contacting the Mississauga Halton Central Intake Program * adults interested in attending a Diabetes Education Program can also contact the Mississauga Halton Central Intake Program at 905-338-2983 ext 4872 to initiate the referral process (self-referral)
