Provides comprehensive geriatric assessment for patients seen in the Emergency Department but not requiring admission and inpatients ready for discharge, who require a timely comprehensive geriatric assessment * goal is to identify and provide a plan for the management of geriatric issues to prevent further Emergency Department visits, avoid unnecessary hospitalizations and reduce length of stay
