- A network of coordinated health care services that provide clinical care and education * a team of health care professionals work collaboratively with patients and families where health concerns may threaten a person's ability to live independently * GAIN team consists of:
- clinical administrative assistants
- nurse practitioners
- registered nurse
- occupational therapist
- pharmacist
- social worker
- Behavioural Support Ontario nurse
- Ontario Health atHome care coordinators
- comprehensive geriatric assessment
- assessment of memory and thinking
- balance, mobility and falls risk
- medication (and supplement) assessment
- general health as well as chronic disease assessment
- mood and mental health
- Eligibility
- Older adults, 65 years and older, living at home/in the community/retirement residence with multiple medical problems such as cognitive impairment, decreased function, falls/risk of falls, impaired mobility, incontinence, multiple medications
