Case Management - Alzheimer's Disease and/or Dementia
- Description
Conducting an assessment, planning, coordinating services and ongoing monitoring to meet the needs, goals and expected outcomes of seniors with Alzheimer's disease and/or dementia * offers two Adult Day Programs for clients suffering from Alzheimer's and other dementia
- Mon, Tues and Wed in Bradford West Gwillimbury
- Balance of Care Adult Day Program (for clients eligible for the Balance of Care project), 3 days a week in Richmond Hill
Clients and caregivers admitted to the day programs also provided support and education from the Alzheimer Societies of York Region and Greater Simcoe County * CHATS also provides an in home dementia respite program for caregivers requiring caregiver respite at a reduced rate for bulk hours (call for more details) * offered in rural area of Georgina, 17th Sideroad (including Schomberg, Pottageville, Kettleby, and Snowball), Perferlaw, Bradford West Gwillimbury, East Gwillimbury, and New Tecumseth.
- Eligibility
- People 55 years of age or older. Specifically for the Richmond Hill BoC ADP, must meet the following eligibility criteria including; on Central LHIN LTC waitlist, and live within the RH/Vaughan catchment area
Residency Requirements: York Region - Application Process
The Alzheimer Society as well as CHATS can assist in completing the referral process. Transportation may be available to assist in accessing programs.
