Description
Social, recreational, and therapeutic activities for older adults who are frail, socially isolated, diagnosed with Alzheimer's disease and other related dementia, or other age related illnesses (stroke, arthritis, Parkinson's) in a group setting for part of the day * helps to support family caregivers who provide day-to-day care

  • wellness education
  • companionship
  • nutritious snacks and lunch
  • medication support
  • assistance with mobility and toileting
  • assessments and on-going care planning and communication
