Free community support services coordinated by Ontario Health atHome to promote successful discharges from either in-patient hospital stays or emergency department visits
- provides individuals access to services for a 4-6 week period to regain strength and independence
Services may include:
- frozen meal delivery
- transportation home from hospital
- transportation for 2 follow up medical appointments
- homemaking services
- home safety assessment
- wellness check ins
- communication with other support services
- access to a 24/7 support line
- Eligibility
Older adults with function impairment being discharged from hospital
- must be medically stable and able to transfer independently
