- Supports for people living with gestational, prediabetes or type 2 diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse, registered dietitian, social worker, endocrinologist and diabetes educator on staff
- non-insulin and insulin injection
- nutritional counseling
- foot screening/assessment
- therapeutic counseling for clients and families
- ongoing management of chronic diseases
- triage service for urgent client illness

- Adults 18 years and older with prediabetes type 2 diabetes
