- Description
The team provides access to primary care for people who may be unhoused/unsheltered and have other complex health concerns such as mental health and substance use and are unattached to primary care or with barriers in accessing appropriate health care support * no health card (OHIP) is required to access the service * team include nurse practitioners and a doctor, registered nurse, social worker, and peer support worker * also offers education and training sessions, group-based activities as well as specialty health clinics to community members
Services include (but are not limited to):- health check-ups
- blood work and tests
- vaccinations
- sexual health testing
- wound care
- smoking cessation
- naloxone and safer drug use equipment
- addictions treatment
- counselling and peer support
- service and specialist referrals
- medical forms

People who have barriers accessing primary care such as no family doctor or nurse practitioner, lost or expired OHIP card * clients who have challenges with social, financial or housing needs * people with mental health or substance use disorder or with multiple physical health concerns
