Education and consultation to service providers offering end-of-life care * sessions includes basic and advance palliative-care training * consultation regarding pain and symptom management
Services include:- case-based palliative care consultations available in-person, by phone or via email for providers
- training and mentorship for front-line providers such as home care agencies, long-term care homes, hospices, day programs, supportive housing, correctional facilities, or volunteer providing hospice palliative care (courses and workshops designed to enhance knowledge)
- resources on a variety of topics including Advance Care Planning, Goals of Care and Health Care Consent, Caregiver Resources, Disease Specific, End-of-Life, Grief and Bereavement, Palliative Approach to Care, Palliative Care Service Delivery Resources, Pain and Symptom Management
