- Description
Exercise program to older adults in their own home * 12-week program * sessions are provided once a week by trained volunteers through a series of 12 simple and progressive exercises * designed to improve strength, balance, and mobility for less mobile older adults while preventing falls
Eligibility criteria:- able to follow instructions or have a caregiver willing to assist and complete exercises independently (not appropriate for mid to late stage dementia/cognitive impairment))
- frailty
- declining mobility
- lives in own home or a retirement home
- unable to leave home to attend community physical activity programs
- not currently receiving physiotherapy
- willing to try Home Support Exercise Program (HSEP)
HSEP may not be suitable for clients with the following characteristics:- unstable health condition, for instance, hypo-tension or angina
- extreme shortness of breath
- any uncontrolled pain
- mild to late stage dementia
