This listing describes how to replace documents, for example, when a wallet is lost or stolen
If you are unable to provide any proofs of identity or guarantor as shown below for replacement of documents, you can establish your identity for replacement of documents by swearing an affadavit before:
- a notary public
- free public Commissioner of Oaths service through
- fee for service public Commissioner of Oaths service through
- contact to determine which information and/or proof of identification you will need prior to your appointment
Replace the following through ServiceOntario
- Oakville's Locations: Trafalgar Village, Dorval Crossing East, Pacific Road, or Staples
- if you are in Ontario, you will need to replace a lost, stolen or damaged driver's licence in-person at a ServiceOntario centre
- bring a personal identification document (verifying your name, date of birth and signature)
- a temporary driver's licence will be issued (new licence will arrive in the mail within 4 to 6 weeks)

- for a red and white health card, you must visit a ServiceOntario centre with a completed Registration for Ontario Health Insurance Coverage form, bring three original documents from the list of qualifying identification documents (one to prove citizenship/immigration status, one to prove residency in Ontario, and one to prove identity
- for a photo health card, and if your name and address has not changed, you can replace it by calling ServiceOntario at 1-800-664-8988
- complete the Ontario Photo Card application form available at a ServiceOntario location and provide proof of legal name and date of birth

- to replace a lost, stolen or damaged birth certificate you can use the online application to order a replacement birth certificate

Replace the following through Service Canada
- Oakville's Location: Trafalgar Village
- Service Canada no longer issues plastic SIN cards
- if you know your SIN, no action is required
- if you need to access your SIN, you can quickly view and print it by signing in to your MSCA
- you can also request a confirmation of your SIN by submitting an application and providing the required documents
- a new number will not be issued
- a new SIN number may be issued only if there is clear evidence that your current SIN has been used fraudulently
