Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff

Diabetes Foot Care - Foot care services for individuals living with Type 2 Diabetes * early intervention, regular monitoring and treatment of foot related concerns

  • DECNET - Diabetes Group Education
  • DECNET - Support, Advocacy & Outreach Programs
  • One-to-one Appointments with a Diabetes Educator (DECNET)
Eligibility
Adults 18 years and older with pre-diabetes and type 2 diabetes

Residency Requirements: North to St Clair/O'Connor/Eglinton, East to Warden, South to Lakeshore, West to Don Valley including Rivertown
