Family Court Support Worker Program

Facilitates a victim's understanding of and passage through the familly court system.

Support provided includes:

• information on the family court process
• assistance in preparing for family court proceedings (documenting history of abuse, accessing Legal Aid)
• debriefing with clients following family court appearances
• referral to specialized services and supports in the community
• safety planning
• court accompaniment to proceedings when appropriate

Schools programs

• EngageMENt 'male ally' program is a conversation-based 4-week program led by a male facilitator that guides male students through discussion around popular and sports culture, through a gender lens, and discusses what 'healthy masculinity' means, wrapping up by planning an experiential or service learning initiative (participants may receive up to 15 volunteer hours toward their Community Involvement requirement)

Full-time co-ed live-in treatment program for youth

• day treatment education program for youth within the program
• highly structured environment where residents work on attaining goals set for themselves in consultation with family, therapist and primary worker in the residence
• families are an integral part of the client's time in treatment and attend weekly sessions with family therapist and live-in treatment worker
• allows youth to develop improved social, coping, and life skills
• residence staff work closely with youth, family and therapist to review goals and treatment plans on a continuous basis
• average length of stay is 3-6 months

A program that provides in-home or on-site personal support for daily living. Wellness promotion is a key component of the program. The goal is to provide support for seniors to encourage long-term, independent living in their own apartments in assisted living communities. Support can include, but is not limited to:

• personal care including showering
• dressing/undressing
• assistance with pre-measured medication
• reassurance
• safety checks

Those who qualify for personal care services may also be eligible for:

• light housekeeping
• laundry
• assistance with meal services
• 24 hour supervision services are also available

Professional services such as nursing are provided through Ontario Heatlh atHome. The Support Services Coordinator for the Assisted Living Program and the Ontario Health atHome Coordinator work closely together to ensure that the seniors' needs are met. Supports for Daily Living Program

• operates at the John R. Rhodes Senior Citizens Residence, 271 Kerr Street, in Oakville

• also operates at Martin House, 189 Ontario Street, Milton
• Halton Region provides this service to eligible residents of Martin House Apartments and eligible individuals living in the surrounding neighbourhood

Emergency Supportive Housing Program:

Housing First:

Justice Supportive Housing Program

• affordable accommodation and flexible supports for people with serious mental illness who have come into conflict with the Criminal Justice System

  • HOMES Peer Specialist, provides peer support to individuals receiving services from the HOMES Program

Help adults live in their communities and fully participate in the activities of daily living * helps adults with disabilities to remain independent * volunteer opportunities available

Supportive housing (Deborah's Home):

• residence for five adults with physical disabilities requiring 24 hour access to personal support services and homemaking support
• tenants who wish to live at Dehorah's Home must be over the age of 16, require a wheelchair accessible living environment, qualified for Rent Geared to Income (RGI) housing, possess valid OHIP, and require access to personal and homemaking assistance 24/7

Attendant Outreach Services:

• a service designed to meet the needs of adults with physical disabilities
• personal and homemaking support is provided to eligible adults in their own homes, schools or workplaces in Halton Region
• services include hygiene, grooming, positioning, dressing and undressing, assist with medication, bowel and bladder care, transfers, meals, housekeeping

Provides transitional housing for South Asian women who are victims of domestic violence, intimate partner violence or gender-based violence

• provides case management and referral/connection with necessary external services
• maximum length of stay is 3 months with an extension for an additional 2 months possible in some cases

• All incoming clients are referred to the program through the Region of Halton's housing list only


• Milton Transitional Housing does not provide emergency or short term shelter but is a bridge from short-term emergency shelter to long-term affordable/market housing at subsidized rental rates for people in housing crisis


• Participants required to undertake counselling and/or training on budgeting, nutrition, employment, as necessary


• Organizes 'The Coldest Night of the Year', annual Milton walk in support of Milton Transitional Housing


• Walk will be held on February 28th, 2026 to raise money for charities serving people experiencing homelessness, hurt, and hunger

Program introduces a temporary housing option for eligible newcomers in the community

• Burlington based apartments and townhouses for families and individuals who are homeless or at risk of homelessness
• Provides temporary accommodation while residents search for permanent housing in Halton
• Placement is up to 12 weeks with a possibility of extension
• Rental Agreement is signed every three months with possibility of extension to up to one year
• Offers a wide range of services, including counseling, and referrals to other assistance programs

Offers on-site 24-hour personal support services for seniors. Provides the Supports for Daily Living program to eligible residents of Martin House and Bruce Apartments along with eligible individuals living in the surrounding neighbourhood (see the boundaries listed below)

Support can include, but is not limited to:

• personal care including showering
• dressing/undressing
• assistance with pre-measured medication
• safety checks

Those who qualify for personal care services may also be eligible for:

• assistance with meal services

Access to health care support at home, at school or in the community * Care Coordinators work with people in the community or leaving hospital to determine what help is needed and arrange for care * care is provided in community settings such as the home, school, residential facility or nursing community clinic * provides information about other community services and refers when appropriate

Mental Health and Addiction Nurses - Assessments, interventions and support to students and families with mental health and or addiction issues * consultation to school staff in regards to mental health and/or addiction issues as appropriate

Placement Services - Determine eligibility for admission to long-term care facilities and prioritize and manage the admission process * support and information is provided to applicants and their families prior to and during the admission process

Nursing Clinics - Nursing services for ambulatory patients at clinics located in the community

Health Care Connect - Refers people who do not have a doctor to a family health care provider who may be accepting new patients * once registered, program participants are assigned to a Care Connector * Care Connectors work with family doctors and nurse practitioners who are accepting new patients * Health811 (get connected to health care 24/7)

• Transportation (gas gift card, taxi voucher, bus ticket, medical transportation, and vehicle expenses)
• Social Integration (access to a cellphone or phone cards)
• Health and Wellness (medication, emergency dental and eyeglasses)
• Housing Start-up Costs (furniture, bedding and mattress)
• Family Supports (childcare, formula and diapers, and school supplies for children/youth)
• Food Security (grocery gift card and baby food)
