Short Description
COMMUNITY SUPPORT PROGRAM provides support to people who have serious, persistent mental illness, who are living in the community in the setting of their choice and who, at times, require support to maintain their housing and independence. This program provides low to moderate intensity support based on individual needs.
SUPPORTIVE HOUSING PROGRAM is designed for individuals requiring long-term intensive support and who are experiencing serious mental health issues. These individuals are homeless or at risk of becoming homeless. The program provides a combination of affordable housing and the ongoing, flexible supports needed to maintain housing. Consumers are linked with services, supports and resources in the broader community. The program supports over 80 housing units in the Niagara Region.
TRANSITIONAL HOUSING SUPPORT PROGRAM: provides therapeutic support to clients with concurrent issues. Life skills instruction and practice is an integral part of the program. The program is time limited to a period of 3 to 6 months. Assistance with finding permanent housing is a component of this program.

Offers wellness suites, which are semi-independent living apartments with care and support available. Supports may include bathing, lifting, transferring, medication reminders, light housekeeping and laundry, and 24 hour safety checks. Wellness suite tenants are provided with meals and access to a 24-hour emergency call bell system.
Free Exercise and Falls Prevention classes are offered on-site. Tenants and local seniors also have access to an on-site Fitness and Wellness Centre.

A one year transitional housing program open to men, women and their children with a focus on families who are homeless due to issues with poverty, mental health, domestic violence, family breakdown and/or physical disability.
During their stay residents are given support through individual meetings with a support worker, support groups and a wide range of practical knowledge and skills training workshops.
Some examples of support groups provided are: life skills, self-esteem, healthy relationships, grief and loss, women's group, men's group and advocacy. After residents move out, they are guided towards ongoing support programs available in the Niagara Region.

Through Familyhome / LifeShare, Ontario families and citizens (home providers) can offer a safe and nurturing environment for adults with special needs. The Familyhome / LifeShare Program encourages personal choice, decision making, independence, and participation in the community. Familyhome Providers are carefully screened through references and police checks, health and fire home inspections, and extensive interviews. Familyhome Providers receive 24-hour on-call support from Case Managers.

Participants enjoy a variety of recreational opportunities including crafts, games, music, picnics, swimming, bowling, etc., based on the individual's interests, strengths and needs. During the summer months and periodically throughout the year, respite services are offered weekly and individuals may utilize this service for up to two weeks. Special conditions apply. Advanced planning is essential and regular, planned respite is recommended as space is limited
